You have a busy practice. Everybody needs you,
yet there isnt enough time in the day to give all your patients the full
attention each of them deserves. PARTNERS IN CARE is a team of professionals
(a geriatrician, a nurse, a social worker, and a chaplain) specializing in the
care of patients who are not quite ready for hospice but who suffer from chronically
debilitating or life-threatening diseases.
Our funding grants us time to help
patients and families understand and comply with your treatment plan.
Our extensive home visits allow us to be your eyes and ears, giving you an in-depth
picture of the day-in day-out psychosocial forces affecting your patients lives.
As with hospice, we counsel and educate your patients
through the difficulties of symptom management, advance directives, and feelings
of hopelessness, fear, and isolation. Unlike with hospice, however, YOUR PATIENTS
DO NOT HAVE TO FOREGO CURATIVE TREATMENT, and neither must they be in their last
six months of life. Anyone with a serious illness can participate. Best of all,
we charge NO FEE FOR OUR SERVICES. Simply complete our

For Partners In Care patients with especially complicated
situations, you can request a palliative
care consultation. This free home visit by Dr. Sherellen Gerhart, our
board-certified geriatrician and palliative care specialist, can give you additional
insights regarding symptom management. Ideal for patients with refractory symptoms
that seem to defy even state of the art treatment, a palliative care consultation
enables you to at least offer improvements in the patients quality of life.
If you would like to learn more about palliative
care, Dr. Gerhart and the PARTNERS IN CARE staff are also available to speak
to your professional organization at no charge. Topics include pain and
symptom management, communicating bad news, practitioner self care for working
with the seriously ill, and ethics and guidelines for determining decision-making
