About Us: Community Nurse and Hospice Care
Community Nurse & Hospice Care (CNHC) is one of the largest providers of home health and hospice services in southeastern Massachusetts. Founded in 1916 by a group of concerned area residents and health professionals, CNHC is a non-profit institution dedicated to rendering comprehensive home care and support to homebound patients in southeastern Massachusetts. Our highly skilled nurses, therapists, social workers, home health aides and volunteers conducted more than 62,000 home care visits in 2007 to 2,000 homebound patients. Our services include, but are not limited to, nursing, physical, occupational and speech therapy, telemonitorin, specialty wound care, IV therapy, psychiatric nursing, medical social work, home health aide and private pay services.
In 1993, CNHC established the first Medicare-certified hospice program in the area to assist patients and their families in the final stages of many types of terminal illness. Our home care professionals are committed to providing compassionate care and support to the seriously ill and their families. Our dedicated hospice team is focused on the comfort needs of those with conditions that cannot be cured. By offering physical, mental, emotional, social, and spiritual support to patients and their families, we help all involved live life fully in the time that remains to them. Last year we provided home care services to over 2,000 patients in the 14 communities that we serve. Our hospice team of 11 staff and 27 volunteers provided over 775 hours of volunteer hours of care, equivalent to $10,370. Our agency offered widow and widower and other bereavement support groups to not only our patients, but also the entire community.
