Our Services: Community Nurse and Hospice Care
Phone Support: We offer 24-hour support by a nurse to our patients. For questions or concerns, call 508-992-6278 and you will be assisted.
Visiting Nurse Services: Most patients prefer to remain independent in their homes for as long as they can. Visiting nurse services at Community Nurse & Hocpice Care include a plan of care developed usually by a Registered Nurse who consults with the patients primary care physician. The physcian will determine with the nurse if other services will need to be provided, such as, rehabilitation visits, telemonitoring for cardiac issues, wound care and dressing changes and a home health aide to assist with activities of daily living. Community Nurse & Hospice Care also offers psychiatric nursing services, a specialty Alzheimer's program. Hospice at Home: Patients and their families have access to a team of highly trained and caring individuals committed to promoting quality of life, patient dignity, and self-determination. In addition to 24-hour telephone support, hospice patients and family caregivers have access to home visits from a:
- Registered nurse to monitor changes, answer questions, and suggest ways to reduce pain and discomfort (physical, mental, emotional, spiritual). Nurses also serve as liaisons with the patient's doctor, facilitating the ordering of services, medication, and equipment that will enhance comfort.
- Home health aide to help with bathing, grooming, shaving, and basic hygiene.
- Volunteer to provide company and companionship for the patient and a much-needed opportunity for the family caregiver to take a break.
- Social worker to assist with emotional issues, family meetings, insurance concerns, and legal and financial decisions, as well as referrals to community programs that offer assistance beyond the scope of hospice.
- Chaplain, to assist with spiritual support based on the patient's own faith tradition. This interfaith minister can help with counseling, prayer, and funeral planning as desired by the patient and family. The chaplain can also serve as a liaison to local faith communities of the patient's choosing.
Bereavement Support Groups: Anyone who is grieving the loss of a loved one is invited to join one of several support groups we offer throughout the week. There is no charge for attending, and no prior affiliation with hospice is required. If you are interested in easing the distress of individuals and their families at the end of life, you can help in many ways:
- Have us speak at your community group, organization, or workplace.
- Become a hospice volunteer.
- Make a donation now.
- Talk to our development office about planned giving.
